Trainee Advanced Care Practitioner

The post holder will work towards gaining ACP accreditation and once able apply advanced clinical expertise, levels of judgement, discretion and decision making within the face to face element of the hospital at home service. As part of the training process they will work alongside more senior/experienced clinical professionals in the team and once able they will act as an autonomous practitioner working independently and will exercise expert clinical practice in assessment, diagnosis, prescribing and development of medical treatment plans for patients with complex presentations and multi pathologies. The role will provide an expert resource, providing a visible clinical profile, having both advisory and clinical input into patient care within the hospital at home service. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. Works collaboratively with the multi-disciplinary team including medical, nursing and allied health staff to manage medical conditions and to plan and implement treatment for illnesses. Once an Advanced Clinical Practitioner, this role will provide education to other staff and students within the clinical area.
